Astaroth has always been one of the more memorable villains in the Diablo 4 universe, but in Season 9, the Lord of Hatred returns in a terrifying new form. Certain builds may be heavily punished by combinations like “Suppressing Aura” and “Frozen Orb.” Players who prepare gear and skills for flexibility, rather than specialization, tend to fare better.

But what makes this fight truly shine is that it isn’t just punishing — it’s also rewarding. Victory over Exalted Astaroth offers one of the rarest drops in the season: the Horadric Jewel. This powerful item is tied to new systems introduced in Season 9 and serves as a status symbol for those who conquer the Escalating Nightmare gauntlet.

The jewel aside, Astaroth also drops unique items, legendary gear, Obducite, and high-value crafting materials. And thanks to the stacking affixes, loot quality is generally higher than standard dungeon content.
